Release Agreement

In consideration for services provided to me by The Mohawk College of Applied Arts and Technology (“Mohawk”), and, in particular, relating to my activities in the Plant-Based Cooking Workshop program (“Program”), I hereby release and forever discharge Mohawk and all of its affiliates, subsidiaries, predecessors, and present and former governors, officers, directors, employees, servants and agents (“Releasees”) from any and all actions, causes of actions, suits, debts, demands, covenants, complaints, contracts, claims, applications, damages, liabilities, statutory rights and obligations, representations, warranties and torts (both intentional and unintentional) whether presently known or unknown, which I may have or may hereafter have against the Releasees for any damages including, but not limited to, death, personal injury, property damage, property theft, arising out of or in any way related to my enrollment and instruction as a student of Mohawk and/or as a participant of the Program.

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during the Program. 

I understand that while participating in this Program, I may be photographed. I agree to allow my photo, video, or film likeness to be used by Mohawk, in accordance with Mohawk’s Privacy Policy. I acknowledge that I have been advised of the risks involved in the Program and the risks associated with my participation in the Program, including risks involved in the provision of transportation provided by Mohawk or by myself using my own personal transportation, and that I have had adequate opportunity to read, understand and consider this Release and the opportunity to obtain independent legal advice with respect there to. I also acknowledge that I am signing this Release voluntarily, without coercion, and without reliance on any representation, whether express or implied, by Mohawk.
