Students are to fulfill the required number of hours as identified by their program for the Field Placement experience in order to meet the expectations.
The student is responsible to contact their Field Placement Specialist and site supervisor/mentor if they are going to be absent or late, prior to the beginning of their shift.
Students are required to make-up any time missed during the Field Placement experience within a designated time period with the approval of their Field Placement Specialist. College classes cannot be missed for the purpose of Field Placement make-up hours/days. Students must coordinate all make-up time with their Field Placement Specialist and inform their Field Placement Supervisor/Mentor.
If the student is working at more than 1 agency during the week, the student is required to include them all on the same submission. Make notes beside the shift hours or in highlights to make note of the different agencies.
Students are to provide an accurate account of each shift and/or any part shift completed at Field Placement. If a shift is missed please record "Absent" in the regular time slot.
The hours that have been submitted by the student will be reviewed and confirmed with your Field Placement Specialist and Placement Site. The student is responsible for the accuracy of their submission and are held accountable for the time recorded on the attendance.

DO NOT try and make changes to a previous submission by re-submitting this form. If you need to make a correction on a previously submitted attendance report please send the correction to your Field Placement Specialist.

Do not make multiple submissions for one week. If you work at more than one site please combine the hours on to one form. Only the 1st submission received for a given week will be accepted and subsequent submissions will not be added to your attendance record.
