Voluntary Indigenous Self Identification Form

Mohawk is committed to providing services to support First Nations (Status/Non-Status), Metis and Inuit students' success. If you're a First Nations (Status/Non-Status), Metis or Inuit student, we encourage you to self-identify.

Students who self-identify with the Indigenous Student Services Office stay connected with Mohawk College and receive the following benefits:

  • Improve and enhance Indigenous programming and services at Mohawk
  • Receive notifications of upcoming events, workshops, and bursary opportunities
  • Eligible for enhanced academic and cultural supports
  • Improve connections with Indigenous communities.

Self-identification is voluntary and confidential, and you won't need to produce any documentation. All collected information is confidential and will not be shared, as outlined in the Freedom of Information and Protection of Privacy Act. The information will only be used to help us provide relevant support, services and resources.

To register with the Indigenous Student Services Office at Mohawk College and receive emails, the following form is required to be completed and submitted.

The information collected will support the planning and delivery of more effective services and activities for our Indigenous students. The information requested on this form is confidential and will be used for student contact, important email information, updates on events and services at the college, and to communicate information that supports Indigenous students socially and financially, for Indigenous learner award consideration, and for ministry statistical reporting. Reports are provided to our Indigenous Education Council (IEC), the Board of Governors, and/or the Ministry of Colleges and Universities (MCU).


Indigenous Identity refers to whether the person reported identifies with the Indigenous peoples of Canada. This includes those who reported being an Indigenous person, who is First Nations (aka “North American Indian”), Métis, or Inuk (Inuit) and/or those who reported Registered or Treaty Indian status, who is registered under the Indian Act of Canada, and/or those who reported membership in a First Nation or Indian band. Indigenous peoples of Canada are defined in the Constitution Act, 1982, Section 35 (2) as including the “Indian”, Inuit, and Metis peoples of Canada (Stats Canada 2015)

In accordance with section 39(2) of the Freedom of Information and Protection of Privacy Act, RSO 2006 and Mohawk College Privacy and Legal Statements, the personal information on this form is collected under the legal authority of the Ontario Colleges of Applied Arts and Technology Act, 2002. The information will be used to provide you with information on a regular basis that is relevant to you as an Indigenous student, and to compile statistical data regarding students who self-identify for required reporting to the MCU.
