How to Change Your Name with Mohawk College
There are several options for updating your name at Mohawk College. Please be sure to take into account how and where you'd like your name to appear, and if you have official government documentation for the change. Below, you will find the outlined options for updating your name with Mohawk.
Keeping your name and address current is essential to ensure Mohawk College can reach you as needed.

Preferred Name Change
To change your preferred name with the college:
- Please visit Student Services Office at your campus or contact the Registrar's Office.
Legal Name Change
To change your legal name with the college:
- Submit the Student Legal Name Change Request form PDF
- Include one piece of official identification showing your new name (driver's license, marriage certificate, etc.) with your submission
- Submit the completed form and identification to the Registrar's Office
- Note: Legal name changes submitted by email must include a a certified copy of your government identification that has been attested by a Guarantor
- Notify the Financial Assistance Office of your new name. Additional documentation may be required to update your OSAP file.
Lived Name Change
- For questions or support related to the lived name change process, you can Book an Appointment or email us at studentnamechange [at] mohawkcollege.ca (StudentNameChange[at]mohawkcollege[dot]ca).
- Student is able to bring a support person to the lived name change meeting. Support person is present in a meeting for moral and/or emotional support and is not to act as an advocate, a representative, or a speaker on behalf of the student. The support person can be an individual, such as a friend, mentor, staff or family member. All support persons are required to maintain confidentiality.
Name Correction for International Students
To correct your name with the college:
- Submit the Name Correction Request Form
- Include one piece of official identification showing your name (passport, etc.) with your submission

- Address changes are done through MyMohawk, and if you no longer have access, please complete the form and we will update your record.
