Request Alternate Format

This form is used to request alternative format for textbooks as well as course content on MyCanvas.

NOTE: If you are not a student and require alternative format for a Mohawk College webpage or event. Please contact alstechnician@mohawkcollege.ca.

In order to request alternative format you will need to be registered with Accessible Learning Services and have alternative format identified on your Accommodation Letter.

Please fill out your personal information below as well as the information requested for they type of alternative format service you select.

You will need to fill out a form for each textbook or course on MyCanvas you require alternative format for.

Textbook Information

Please provide the information below for the textbook you are requesting in alternative format.

Please enter the title of the textbook.
Please enter the edition of the textbook.
Please enter the name of the author(s) of the textbook.
Please enter the name of the publisher of the textbook.
Please enter the 13-digit ISBN for the textbook. This can usually be found on the back of the book near the barcode or on the first couple of pages. This number starts with 978. Please ensure it is the number on the book not on a sticker or wrapping.

Proof of Ownership

Below you can provide your proof of ownership for the textbook. If you have a copy of the receipt or invoice you can upload it. If you do not have a copy of your receipt or invoice please provide the information requested.
