Banner forms access
Request creation of a Banner account and access to Banner forms

Managers can request access to Banner on behalf of their employees.
Change e-mail display name
Employee name changes: see Legal or Preferred Name Change under Personal Information on MyMohawk (new window) through Human Resources.
Student name changes: see Name or Address Change (new window) through the Registrar's Office.
Cognos Permissions and Distribution List Request
Request to grant or revoke employee access to Cognos, report distribution lists or modify existing access permissions.
Available to: Employees.
Turnaround time: 2 business days.
Lead time: Request 2 business days in advance.
Include the following details in your request.
- 9-digit employee number: for the person requiring access or removal.
- Employee's Full name: Full name for the employee requiring access or removal.
- Employee's Department: Department name for the employee requiring access or removal.
- Employee's Supervisor: Direct supervisor approving access or removal.
- Access Permissions Type: Approve or revoke
- Access Permissions Purpose: Describe the purpose for the permissions request.
- Sponsored Systems Access: Provide details on an existing employee account that access permissions can be copied.
- Report(s)/Distribution List(s): Describe or list the desired reports or distribution lists to include in the request.
Department or special purpose account
Request a department account or special purpose account
Available to: Employees, students or contracted third parties that are sponsored by a Mohawk employee.
Turnaround time: 2 business days
Includes shared calendars, shared mailboxes, and temporary accounts for student employees or contracted third parties who are acting as employees for a limited time.
Employee access removal
Remove access and account privileges from persons that are no longer employed by or contracted to the college.
Available to: Admin employees, Human Resources representatives
Remove access to Banner, FAST, e-mail, eLearn, phones, and other college systems.

Request access to the Employee VPN service.

The employee VPN is a secure connection to the Mohawk network, allowing you to work safely off-campus when connected to public Wi-Fi hotspots or your home network. For usage instructions, see MyMohawk.
Enable Self-Service access
Request to re-enable access to self-service when "Web Access Disabled" error appears.
Available to: Employees, students, applicants
After five failed login attempts, access to self-service modules is disabled for security. Request to have access re-enabled. Include the following details in your request.

FAST Suite Finance
Request access to FAST Suite Finance
Available to: Employees

FAST suite access for finance is granted upon request by a manager on behalf of an employee. Include the following information in your request e-mail.

- Type of Access Required: FAST finance
- Funds: (List of specific Funds this access is requested for)
- ORGs: (List of specific ORGs this access is requested for)
Long-Term Local Admin Permission
Receive local admin permissions for a Mohawk College owned devices that runs Windows 10 or Apple macOS for an approved extended period.
Available to: Mohawk employees, non-Mohawk users such as contractors, consultants, researchers, volunteers, or other workers
Turnaround time: 2 business days
This service can only be requested through the IT Self-Service Portal. Eligible users will be able to make changes and/or access to other accounts, roles, configurations, and install authorized software 1 device as outlined in the Local Administrator Policy.
MS-Access application permissions
Add, change, or revoke permissions for MS Access tools such as Academic Reporting Tool (ART), Class Designation Chart (CDC), Student Enrollment Manager (SEM)

Managers can request access to MS-Access applications on behalf of their employees.
MyMohawk access
Request access to MyMohawk for non-employee business partners or sponsored contractors
Available to: Managers
Turnaround time: 2 business days
Current students, employees, and applicants can automatically access MyMohawk and do not need to request.
Managers can request MyMohawk access for non-employee business partners or contractors sponsored by them.
Network folder access
Request or change permissions for a specific network folder
Available to: Employees
Turnaround time: 2 business days
Access changes must be approved by a manager or submitted by a manager on behalf of an employee. Include the 9-digit employee number and the folder name in your request.
On-Demand Local Admin Permission
Receive local admin permissions for a Mohawk College owned devices that runs Windows 10 or Apple macOS for 24 hours at the time of receiving access permission.
Available to: Mohawk employees, non-Mohawk users such as contractors, consultants, researchers, volunteers, or other workers
Turnaround time: 2 business days
Eligible users will be able to make changes and/or access to other accounts, roles, configurations, and install authorized software on that device.
PCI System or Room Access

Request access to PCI-audited secure rooms or technology related to credit card payment channels.
Available to: Employees, requested by manager.

Must be requested by a manager. Requests from any other party will be denied.
Third party remote access
Request VPN access for non-employee users
Available to: Employees
Turnaround time: 3 business days
A manager can request VPN access to a sponsored non-college user or contractor. Complete the Third Party Access Form and submit it to the IT Service Desk by e-mail.
